form with copy of the credit card (front and back)
and photo ID to us
so that we can promptly process your order. Please
also follow
instructions included in the form.
Your completion of this authorization form helps us
protect you, our
valued customer, from credit card fraud. All the
information obtained
on the form will be kept strictly confidential by
Johnny Dang & CO.
In addition, you can completely eliminate the
verification process by
using wire transfer, money order or cashier's check
as your method of
payment. Please let us know whether you would like
us to send details
about wire transfer (please specify whether you
would like to use Bank
to Bank or Western Union type service) so that we
can email details.
